Information to be Provided

See the below lists of information to be emailed to the VITA Center after your appointment is scheduled (some items may not apply to you).

U.S. Citizens or Resident Aliens:

Completed Intake/Interview & Quality Review Sheet (IRS Form 13614-C)
Completed Virtual VITA/TCE Taxpayer Consent (IRS Form 14446)
All Forms W-2, 1099-MISC, etc. and information for all other sources of income you received during the tax year
Forms 1098-T for the current year and prior year as well as receipts for books and equipment purchased for educational purposes (if you had not been in college over four years)
Interest and dividend statements from banks (Forms 1099)
Total paid for daycare provider, provider's EIN
Social Security cards for you, your spouse, and dependents or ITIN assignment letters for any such individuals
Birthdates for you, your spouse, and dependents
Documentation of any donations made to U.S. charities
Documentation of property taxes paid on your home or personal property (vehicles)
All Forms 1095, Health Insurance Statements and Health Insurance Exemption Certificate, if received
Last year's federal returns (if possible), if you filed last year
Bank routing and account numbers for direct deposit
IRS Letter – Notice 1444 Your Economic Impact Payment

Non-Resident Aliens [generally, most international students]:

Completed Non-Resident Alien Intake/Interview & Quality Review Sheet (IRS Form 13614-NR)
Completed Virtual VITA/TCE Taxpayer Consent (IRS Form 14446)
Passport
Form I-94, Arrival and Departure Record
Photo ID for all taxpayers to be included on your tax return
Birthdates for you, your spouse, and dependents
Forms W-2, W-2G, 1099-R,1099-Misc from all employers
Forms 1042-S, Foreign Person's U.S. Source Income Subject to Withholding
Forms 1098-T
